This role offers the flexibility of a remote work setting, allowing you to engage with clients directly in their homes across Saint Louis. Our staff operates from a virtual community health clinic model, utilizing technology to coordinate care and collaborate with interdisciplinary teams. While the primary hours are Monday to Friday from 8:00 AM to 4:30 PM, flexibility is essential. Occasionally, evening and weekend hours may be required to meet client needs, alongside on-call availability for urgent situations.

• Conduct thorough assessments of clients' psychosocial needs, analyzing emotional, mental, and social factors that influence their overall health and welfare.
• Develop and implement tailored care plans in collaboration with seniors and their families, respecting individual goals and preferences while addressing their unique circumstances.
• Provide professional counseling and emotional support, helping clients navigate life transitions and challenges with compassion and understanding.
• Advocate for client rights, ensuring access to necessary community resources and support services that enhance seniors' quality of life.
• Collaborate efficiently with an interdisciplinary team, including nurses, caregivers, and other healthcare professionals, to optimize the coordination of care.
• Conduct regular home visits to monitor clients' well-being, evaluate home safety conditions, and implement adjustments in care as needed.
• Play a mediating role in family dynamics by facilitating communication and conflict resolution among family caregivers involved in the clients’ care.
• Educate clients and their families about available community resources and support networks, encouraging social engagement and community involvement.
• Maintain comprehensive and accurate documentation of client interactions, assessments, care plans, and progress notes to comply with legal and ethical standards.
• Stay abreast of developments in social work practices, community resources, and healthcare policies to ensure the delivery of informed and compliant services.

The primary focus of this position is on seniors and elderly clients who need support for physical and emotional well-being while living in their own homes. The Home Care Social Worker will engage with clients experiencing a range of challenges, including chronic illness and life transitions such as moving from independence to needing assistance, thereby addressing their needs holistically.

• Bachelor’s degree in Social Work from an accredited institution.
• Valid state licensure as a Social Worker (e.g., LCSW, LMSW, or equivalent).
• A minimum of two years' experience in social work, ideally within the home care or geriatric sectors.
• Deep understanding of social work principles, age-related issues, and available community support resources.
• Exceptional skills in assessment, counseling, and the effective application of problem-solving techniques.
• Demonstrated empathy, compassion, and the ability to cultivate relationships with seniors and their families.
• Strong communication and interpersonal skills to convey information effectively and sensitively.

• Experience working collaboratively within an interdisciplinary team in a healthcare setting.
• Knowledge of healthcare regulations and compliance standards.
• Familiarity with electronic health record systems to document client interactions efficiently.

The salary range for this full-time position is between USD 60,000 and 100,000 annually, commensurate with experience and qualifications. In addition to competitive pay, we offer a robust benefits package that includes:
