Key Responsibilities

  • Provide empathetic counseling services within your scope of training and licensure, ensuring a tailored approach to meet individual patient needs.
  • Assist patients seeking transplant through educational and goal-directed counseling, addressing their psychosocial requirements.
  • Actively engage with community resources to facilitate optimal treatment outcomes for patients, ensuring they have access to essential services like home health and vocational rehabilitation.
  • Continuously update knowledge of local vocational and educational resources, helping patients maintain or achieve employment and educational goals.
  • Engage patients in discussions about advance directives, aiding them in understanding their options and facilitating necessary conversations with healthcare teams.
  • Collaborate with an interdisciplinary team to assess patients' psychosocial needs, identifying strengths and areas for growth that impact rehabilitation.
  • Participate in care planning processes, working closely with patients and healthcare professionals to determine effective interventions for rehabilitation and treatment goals.
  • Utilize established methodologies and assessment tools to identify and address barriers to treatment, enhancing the patient's journey toward mental wellness.
Patient Population / Client Focus

Our patient population primarily includes individuals undergoing dialysis treatment and those seeking assistance in mental health and psychosocial rehabilitation. You will work with diverse clients, including those facing complex emotional challenges related to their medical conditions. The focus is on creating a supportive environment where patients can express their needs and work towards achieving their health and wellness goals.

Qualifications
  • Must hold an active Social Work License as per state regulations (LMHC, LMFT, LICSW).
  • No prior history of medical malpractice claims, confirmed by a third party.
  • Valid authorization to work in the United States.
  • A Master’s degree in Psychology, Counseling, Social Work, or a related field.
Preferred Experience
