• Deliver direct clinical services to a varied caseload of clients exhibiting symptoms of major mental illnesses or severe emotional disorders, utilizing a client-centered approach.
• Conduct comprehensive assessments using evidence-based screening tools to determine necessary treatment interventions, including individual, group, and family therapy plans tailored to client needs.
• Create and implement integrated care plans that are holistic and collaborative, ensuring effective communication among clients, their families, and other providers.
• Participate actively in interdisciplinary team meetings to ensure alignment on client goals and treatment advancements, working collaboratively towards common objectives.
• Monitor and document client progress, facilitating timely adjustments to treatment plans based on ongoing evaluation and feedback.
• Provide crisis intervention and support as required, utilizing established protocols to manage emergencies effectively.
• Participate in the on-call rotation to ensure continuity of care during evenings and weekends.
• Engage in continuous professional development to enhance clinical skills and contribute to maintaining high ethical standards and adherence to the organizational code of conduct.

This position primarily focuses on serving clients in hospice care, dealing with end-of-life issues and the complexities associated with grief, loss, and severe emotional distress. You will work with a diverse patient demographic, including those from various cultural backgrounds, and emphasize sensitivity and respect for each individual’s unique experiences and needs. Your expertise will help facilitate meaningful conversations and support systems for both clients and their families during critical times.

• Must possess a valid license as a Clinical Social Worker (LCSW) in the state of Virginia.
• A minimum of two years of clinical experience in a healthcare or social services setting, preferably working in mental health or hospice environments.
• A Bachelor’s Degree in Social Work or a related field is required; a Master's Degree is preferred.
• Strong commitment to ethical clinical practice and the ability to maintain confidentiality in all interactions.

• Experience working within a hospice or palliative care setting.
• Familiarity with telehealth platforms and digital tools for client assessment and therapy delivery.
• Demonstrated ability to implement evidence-based practices tailored to diverse populations.
• Knowledge of community resources available for clients facing mental health challenges.

This role offers a competitive salary ranging from USD 70,000 to 85,000 per year, commensurate with experience.
