• Complete thorough bio-psychosocial assessments to identify barriers that impact patients’ health and treatment adherence.
• Work closely with medical staff to develop interdisciplinary care plans that enhance patient outcomes and ensure continuity of care.
• Utilize evidence-based therapeutic techniques to create individualized treatment plans and implement both individual and group therapy sessions.
• Document all clinical activities accurately and in accordance with department standards, contributing to the informed medical record of each patient.
• Educate patients and their families about mental health issues and the resources available to them, promoting understanding and acceptance of treatment recommendations.
• Conduct advocacy work to connect patients with community resources that can support their social, emotional, and practical needs.
• Maintain a commitment to adhering to the NASW Code of Ethics and uphold professional standards throughout all interactions.

This role primarily supports patients within a hospice care environment, focusing on end-of-life issues and the psychosocial complexities that arise during this sensitive phase of care. You will engage with a diverse population, including patients of various ages, socioeconomic backgrounds, and cultural identities. Your interventions will be designed to enhance their overall quality of life while navigating the challenges associated with terminal conditions.

• A Master’s Degree in Social Work from an accredited institution.
• A current and valid LCSW license issued by the State Board of Social Work Licensure.
• In-depth knowledge of clinical social work practices and ethics, coupled with strong analytical and therapeutic skills.
• Comprehensive understanding of aging processes, developmental stages, and cultural considerations that inform care delivery.

• At least 1-2 years of clinical experience in mental health or healthcare settings, specifically with a focus on hospice populations.
• Proven experience providing treatment and support to diverse patient demographics, including children, adolescents, and adults.
• Familiarity with outpatient treatment modalities and community resources available for patients.

We offer a competitive salary ranging between $85,000 to $100,000 annually, based on experience and qualifications.
