Life Solutions Services is seeking a dedicated Licensed Professional Counselor (LPC) to join our growing team in Westminster, Colorado. • Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues as a key member of the healthcare team.
• Conduct comprehensive patient intakes that focus on diagnostic assessments and functional evaluations, providing actionable recommendations for treatment goals.
• Deliver brief, focused interventions tailored to patients requiring mental health services, ensuring timely and effective care.
• Advise primary care providers on the suitability of mental health interventions within the primary care setting and recommend referrals to specialized mental health facilities when necessary.
• Initiate follow-up calls or meetings with patients to monitor progress, adjust treatment plans, and provide ongoing support.
• Develop relapse prevention strategies and assist patients in achieving stable functioning through proactive intervention.
• Monitor and coordinate the delivery of behavioral health services, facilitating communication among different care providers with patient consent.
• Document all patient interactions, progress notes, and diagnostic information comprehensively and accurately.
• Maintain ongoing communication with primary care providers regarding patient needs and collaborate in formulating effective treatment plans.
• Promote preventive measures and advocate for lifestyle changes that support mental wellbeing in patients with co-existing physical disorders.
• Equip patients with self-management skills and educational resources to empower their active participation in treatment and recovery.
• Provide consultation to clinic management regarding behavioral health programs and outcomes evaluation.
• Support patients in adhering to medical treatments initiated by primary care providers by providing coping strategies for medication management.
• Comply with the Code of Ethical Conduct for Clinical Professionals while carrying out assigned duties.

• A minimum of a Master’s Degree in Clinical Counseling or a related field is required.
• Current Licensed Professional Counselor (LPC) License.
• In-depth knowledge of mental health assessment processes, treatment planning, developmental psychopathology, and effective therapy methodologies, including systems and family therapy.
• Ability to conduct patient evaluations and provide individualized care plans based on thorough assessments.
• Excellent communication skills, both verbal and written, to engage effectively with patients, families, and interdisciplinary teams.
• Capacity to maintain detailed clinical records and prepare relevant treatment reports.
• Strong interpersonal skills to foster collaborative relationships with colleagues and community resources.
• Mental and physical health sufficient to meet job demands and provide quality patient care.

• Bilingual proficiency in English and Spanish is preferred but not mandatory, enhancing our ability to serve a broader patient demographic.
• Familiarity with telehealth platforms and workflow processes to facilitate efficient and effective remote care.
• Previous experience in a multi-disciplinary healthcare setting is advantageous.

This full-time position offers a competitive salary ranging from USD 87,000 to 105,000 annually, depending on experience and qualifications.
