A Brief Overview
The Substance Use Disorder (SUD) Coordinator provides direct substance use disorder services to patients across the hospital setting. This position is responsible for conducting comprehensive assessments of patients' substance use needs and identifying appropriate interventions. The SUD Coordinator completes substance use screenings and assessments, interviews patients and family members, and collaborates with interdisciplinary staff and community resources to evaluate the impact of substance use on the patient's physical, psychological, and social functioning. Based on assessment findings, the SUD Coordinator works collaboratively with patients, families, and the treatment team to develop and implement individualized intervention, placement, and discharge plans that support continuity of care and recovery.
Education Qualifications

  • Master's Degree M.A., M.S. or related field Required


Experience Qualifications

  • 2 years Experience in addiction or counseling services or related field. Preferred
  • Medical and/or Social Work experience Preferred
  • Experience in substance use disorder cases, assessments, and referrals to all levels of substance use disorder treatments. Preferred
  • Experience in group work, case work, and community methodologies Preferred
  • Customer service experience Preferred

What you will do

  • Conducts substance use screenings, comprehensive assessments, and determines appropriate discharge placement
  • Coordinates substance use disorder services for patients in the Emergency Department and other hospital units as needed
  • Utilizing evidence-based practice strategies and techniques to increase engagement of patients and improve care outcomes for high utilizers of acute services
  • Interviews patients and family members, reviews medical and psychosocial records, and consults with physicians, nursing staff, and other professionals to assess patient needs and condition
  • Provides crisis intervention, counseling, and support to patients and family members when indicated
  • Collaborates with interdisciplinary hospital staff to promote effective management and treatment of patients with substance use disorders
  • Refers patients to appropriate internal and external services, including detoxification, medical evaluation, treatment programs, social services, and recovery support resources
  • Develops, coordinates, and facilitates placement and discharge plans to ensure appropriate follow up care and continuity of services
  • Maintains professional levels of productivity, coordinates, and prioritizes patient care related activities in an efficient and effective manner.
  • Provides problem solving and leadership in the development of new services which respond to patient care demands.
  • Maintains confidentiality and complies with professional ethics according to professional, departmental and organizational standards.
  • Actively participates and initiates when appropriate during patient care conferences.
  • Provide patient education regarding diagnosis and treatment.
  • Provide crisis and/or brief therapeutic interventions for individual, family and group.
