Your primary focus will be on a diverse patient population that includes individuals facing terminal illnesses, their families, and caretakers. You will support patients through counseling, offer emotional support, facilitate family discussions, and advocate for their wishes and preferences throughout the end-of-life journey. Your role is crucial in helping patients achieve a peaceful and dignified quality of life during challenging times.

• A Master’s degree in Social Work from an accredited institution is required.
• Current and valid LCSW license in the state of Illinois.
• Minimum of three years of clinical experience, particularly specializing in psychotherapy and hospice care.

• Outstanding communication and interpersonal skills to interact effectively with patients, families, and other healthcare professionals.
