We are looking for an RN to deliver patient centered health coaching and chronic disease management via chat, phone, and video. This role includes triaging symptoms, providing education on diabetes, hypertension, hyperlipidemia, and weight management, coordinating care with providers and cross functional teams, and documenting patient care accurately in a telehealth setting.

What you'll do
  • Deliver patient centric health coaching that integrates medical needs with patient preferences.
  • Triage patient reported symptoms and side effects according to internal protocols, escalating to Providers when appropriate.
  • Provide comprehensive diabetes, hypertension, hyperlipidemia, and weight management care and education via chat, phone, and video.
  • Coordinate care seamlessly between patients, providers, support teams, and cross-functional clinical team members.
  • Create consults that align with 9amHealth's internal protocols for Providers.
  • Document patient care accurately and consistently to meet internal standards.

What you'll need
Qualifications
  • Strong empathy, communication, and patient-centered care skills
  • Bachelor of Science in Nursing (BSN) required
  • Active Registered Nurse license in good standing, with ability to obtain additional licensure if needed
  • Certified Diabetes Care and Education Specialist (CDCES) or willing to pursue if not currently certified
  • 3+ years of direct patient care in an ambulatory or telehealth setting
  • Adaptable, self directed, and comfortable working independently
  • Proficiency with digital tools for care delivery
  • Flexibility to participate in rotational weekend or holiday coverage
  • A HIPAA compliant, distraction free workspace

This role offers a salary of up to $100,000, depending on experience.
