• Conduct comprehensive ongoing assessments to tailor treatment plans to each client's needs, ensuring personalized care.
• Actively communicate and collaborate with supervisors, direct care staff, families, guardians, and relevant stakeholders concerning the progress of individuals under your care.
• Establish a tentative schedule for appointments, home visits, and observations to systematically monitor client advancement and overall program effectiveness.
• Coordinate and execute in-home observations, chart reviews, and staff interviews to support data-driven analysis and effective care solutions.
• Guarantee the implementation of treatment plans is monitored consistently, allowing for adjustments based on patient feedback and progress.
• Participate in interdisciplinary team meetings and relevant discussions, contributing to a collaborative approach to patient care.
• Prepare detailed monthly and quarterly progress notes that document client developments and outcomes, ensuring continuity of care.
• Organize and facilitate training sessions to keep all staff updated on program plan implementation and documentation requirements.
• Engage in follow-up interviews, home visits, and ongoing assessments to provide continued support for clients' evolving needs.
• Maintain accurate and accessible client records, ensuring sensitive information is readily available to authorized personnel.
• Act as a liaison between the public, mental health communities, and clients to facilitate the achievement of care development goals.
• Assist in the design and refinement of procedures for training materials aimed at enhancing program delivery for individuals.

The successful candidate will work with a diverse patient population, including both adults and youth facing a range of mental health challenges. • A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• Active, unrestricted LCPC licensure in the state of Maryland.
• A minimum of two years of counseling experience, particularly within a clinical context, is essential.
• Specialization in family therapy, couples therapy, substance misuse, or trauma is highly preferred, enhancing our ability to serve specific client needs.
• Thorough understanding of legal and ethical standards in mental health practice.
