This full-time, remote position is focused on providing tailored support and guidance to seniors and their families. Work Environment

This position primarily focuses on remote work, giving the selected candidate the flexibility to perform responsibilities from home while maintaining regular communication with clients and families. Our operational hours are Monday to Friday, from 8:00 AM to 4:30 PM. Additionally, there may be occasional requirements for evening or weekend hours to meet client needs. Therefore, a commitment to flexibility in scheduling and on-call availability is essential.

Key Responsibilities
  • Conduct comprehensive assessments of seniors’ psychosocial needs, incorporating emotional, mental, and social factors critical to their care.
  • Collaborate closely with seniors and their families to develop individualized care plans that prioritize specific goals and preferences.
  • Provide regular emotional support and counseling to clients, assisting them through life transitions and addressing common challenges faced by seniors.
  • Advocate for clients’ rights and help them access vital community resources, ensuring they receive appropriate services tailored to their needs.
  • Engage with interdisciplinary teams, which typically include healthcare professionals, caregivers, and nurses, to coordinate and enhance overall care.
  • Conduct regular client home visits to monitor living conditions, address safety concerns, and evaluate the effectiveness of support systems in place.
  • Facilitate dialogue among family members involved in the senior client's care to resolve conflicts and promote cooperation.
  • Educate clients and their families about community resources and support groups designed to enhance social engagement.
  • Maintain accurate records of client assessments, care plans, and progress notes, ensuring compliance with ethical standards in social work.
  • Stay updated on best practices in social work, including current policies and regulations, to provide the best possible client outcomes.

Qualifications
  • A Master’s degree in Social Work (MSW) from an accredited institution.
  • Current Maryland state licensure as a Licensed Certified Social Worker (LCSW-C).
