Job Title: Healthcare Consultant I - FLA - (Cutler Bay) Address: Cutler Bay, FL Job Description Coordinates case management activities for Medicaid Long Term Care/Comprehensive Program enrollees. Utilizes critical thinking and judgment to collaborate and inform the case management process. Conducts comprehensive evaluations of members using care management tools. Conducts multidisciplinary reviews to achieve optimal outcomes and escalates quality of care issues as needed.

Provides coaching and support to empower members in their healthcare decision-making. Ensures compliance with regulatory and accreditation guidelines during monitoring and documentation of care. Critical Information Work from home position, candidates must reside in Cutler Bay, FL. Remote training via Microsoft Teams for approximately 4-6 weeks. Candidate will travel approximately 75% of the time within the region to meet members. Monday - Friday, 8 am - 5 pm work schedule. Education/Licenses Needed Bachelor's degree required, preferably in Social Work or a related field. High School Diploma or GED is required.
