Stay Close to Home. Make a Big Impact.

One of the biggest frustrations home health nurses have is spending too much time driving.

We've designed this position around a dedicated primary territory, allowing you to spend more time caring for patients and less time traveling across the metro.

You'll become the familiar face your patients look forward to seeing while building lasting relationships throughout the communities you serve.

Why This Territory Works

  • A consistent primary service area across the western Twin Cities.
  • Build long-term relationships with patients, families, and referral partners.
  • Less windshield time and more meaningful patient care.
  • Weekend and on-call visits remain within approximately 35 miles of your home, helping keep travel manageable.
  • Independence in the field with strong clinical support whenever you need it.

A Few Perks We're Especially Excited to Offer

  • $6,000 in first-year bonuses.
  • Free access to our MedBridge Education Hub for continuing education and professional development.
  • Licensure reimbursement when it's time to renew.
  • Annual scrub reimbursement.
  • Mileage reimbursement between patient visits.
  • Cell phone reimbursement for eligible clinicians.
  • Comprehensive medical, dental, vision, PTO, paid holidays, and 401(k).

Why Home Health Care, Inc.

We're proud to be one of Minnesota's longest-standing home health organizations, serving patients for more than 30 years.

  • Manageable caseloads that support exceptional patient care.
  • Leadership that understands the realities of field nursing.
  • A collaborative clinical team that's always available.
  • Locally owned with a culture built around supporting clinicians.

What You'll Do

  • Manage a home health caseload throughout your assigned territory.
  • Complete OASIS assessments and individualized care plans.
  • Deliver skilled nursing care and patient education.
  • Collaborate with physicians and interdisciplinary team members.
  • Help patients remain safe, healthy, and independent at home.
  • Maintain timely and accurate clinical documentation.

What We're Looking For

  • Active Minnesota Registered Nurse (RN) license.
  • 3+ years of RN experience (home health experience preferred).
  • Strong assessment, communication, and organizational skills.
  • Ability to work independently while managing a field-based schedule.
  • Reliable transportation.
