Key Responsibilities
  • Conduct thorough intake and psychosocial assessments to understand patients' needs and develop individualized care plans.
  • Collaborate with the Chemical Dependency Recovery Program/Specialized treatment team to develop, present, and revise comprehensive treatment plans based on client feedback and clinical best practices.
  • Actively participate in clinical case conferences to discuss treatment progress, share insights, and formulate innovative strategies for patient engagement.
  • Lead educational lectures focusing on chemical dependency, offering valuable information and resources to patients and their families.
  • Deliver targeted counseling services, both individually and in group settings, to support clients managing chemical dependency and related issues.
  • Provide family-oriented therapeutic services, including adolescent, significant other, family, and multi-family therapy, ensuring a supportive environment for healing.
  • Offer robust case management services aimed at connecting patients with essential resources and community support systems to enhance their recovery journey.
Patient Population / Client Focus

This role specifically focuses on patients facing serious illnesses, including those receiving hospice care. You will be working with individuals and families navigating the complexities of end-of-life issues, emotional distress, and the psychological impacts of terminal diagnoses. Your expertise will be vital in helping them process their experiences, access critical resources, and foster resilience during challenging times.

Qualifications
  • A masterโ€™s degree in social work (MSW) or a closely related field is mandatory.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.
  • At least two years of post-graduate experience in supervised addiction medicine therapy within the last five years.
