This LMSW role operates primarily in a remote setting, allowing for flexible scheduling while still requiring direct engagement with clients in school-related environments. Working hours are typically Monday to Friday from 8:00 AM to 4:30 PM, with occasional evenings or weekends as required to meet client needs. This flexibility ensures that you can effectively manage client visits and maintain continuity of care. • Conduct comprehensive assessments to evaluate seniors' psychosocial needs, taking into consideration emotional, mental, and social factors influencing their quality of life.
• Collaborate closely with clients and their families to develop and implement personalized care plans that align with their specific goals and preferences.
• Provide individual and group counseling sessions, offering emotional support and guidance to seniors navigating life transitions, such as bereavement or changes in health status.
• Advocate effectively for clients' rights, ensuring access to necessary community resources and services.
• Work alongside an interdisciplinary team, which includes nurses, healthcare professionals, and caregivers, to coordinate comprehensive care for clients.
• Regularly visit clients to monitor their living conditions, provide support, and address any evolving needs or concerns.
• Facilitate healthy communication among family members involved in the care of clients, helping to resolve any conflicts that may arise within caregiving dynamics.
• Educate clients and their families about available resources, support groups, and programs aimed at enhancing social engagement and overall well-being.
• Maintain accurate documentation of all client interactions, assessments, and care plans in compliance with organizational and legal standards.
• Stay informed about current best practices, policies, and regulations in social work to ensure adherence to ethical standards in all client interactions.

Your primary focus will be seniors within our community who may be facing various challenges related to aging, health issues, and changes in social conditions. Understanding their distinct needs is crucial in crafting effective care strategies. You will work to empower seniors to engage actively in their care, providing them with the tools and knowledge they need to improve their emotional and social health.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of relevant experience in social work, preferably within a geriatric or community-based setting.
• Proficient knowledge of social work principles, aging-related issues, and available community resources.
• Strong assessment and counseling skills with an ability to empathize and build rapport with seniors and their families.
• Excellent communication and interpersonal skills conducive to collaboration with an interdisciplinary team.
• Familiarity with electronic health record (EHR) systems and the ability to manage digital documentation effectively.

Experience working specifically with senior populations and in community centers is preferred. Candidates should demonstrate proficiency in managing complex emotional and social issues, showing a keen understanding of the dynamics involved in geriatric care.
