Work Environment

Key Responsibilities

• Act as a liaison between families and outpatient healthcare providers to facilitate continuous, high-quality care for clients.

• Interact with insurance companies to coordinate pre-certifications and manage utilization, ensuring that client access to needed services is streamlined and efficient.

• Participate actively in discharge planning and aftercare initiatives, ensuring clients transition smoothly from hospice care to their subsequent treatment phases.

• Provide therapeutic meal support, guiding clients to foster a positive relationship with food through modeling and coaching.

• Deliver empathetic and affirming care tailored to the specific identities and needs of each client, addressing their individual circumstances with sensitivity and respect.

Patient Population / Client Focus

In this position, you’ll primarily work with older adults dealing with serious, chronic, and terminal illnesses. Understanding the unique challenges faced by this population is crucial for delivering appropriate support. You'll engage with clients in various stages of their health journeys, helping them navigate the complexities of end-of-life care. Adequate training and familiarity with geriatric issues will enhance your ability to connect and promote positive outcomes in mental health and emotional well-being.

Qualifications

• Must possess a Master’s degree in Social Work from an accredited institution.
• Hold a valid LCSW license in the state of Nevada.
• Experience working with older adults is essential.
• Demonstrated strong clinical and diagnostic skills, with the ability to assess and address complex emotional situations.
• Exceptional communication and interpersonal skills for effective interaction with clients, families, and healthcare teams.
• Capability to work independently while also collaborating within a team-oriented environment.

Preferred Experience

• Experience with interdisciplinary care teams and familiarity with hospice and palliative care settings.
• Knowledge of mental health diagnostics, treatment planning, and care coordination specific to elderly populations.
• Proficiency in utilizing electronic health record (EHR) systems for documentation and care planning.

Compensation and Benefits

We offer a competitive salary range of USD 90,000 to 105,000 annually, commensurate with experience and qualifications. In addition, we provide a comprehensive benefits package that includes:
