Work Environment

This position allows for a fully remote work environment, providing you with flexibility while supporting patients within the hospice center. Your role will involve collaborating with healthcare professionals in various settings, ensuring cohesive care for individuals facing end-of-life challenges. Although this role is remote, team collaboration is enhanced through regular digital meetings and virtual case consultations.

Key Responsibilities

• Conduct thorough mental health assessments of patients and their families, taking into account their personal histories, presenting concerns, and unique circumstances.
• Develop individualized treatment plans in partnership with patients, focusing on their mental health needs, emotional well-being, and overall integration into the hospice care model.
• Implement evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and grief counseling, tailored to address a wide spectrum of mental health concerns.
• Facilitate both individual and group therapy sessions, fostering a supportive environment for patients to express emotions and develop effective coping mechanisms.
• Collaborate with other health professionals, including physicians and nursing staff, to ensure a holistic approach to patient care and facilitate seamless communication.
• Provide crisis intervention and support during emotional emergencies, employing de-escalation techniques, and making referrals for more intensive care when necessary.
• Educate patients and families about mental health issues, treatment options, and community resources to empower them in their care journey.
• Maintain meticulous documentation, including assessments, treatment plans, and progress notes, ensuring compliance with relevant regulatory and ethical standards.
• Participate actively in team meetings, contributing insights from your expertise and experiences to enhance the team's collective knowledge.
• Stay updated on best practices, policies, and ethical guidelines pertinent to social work in end-of-life care settings.

Patient Population / Client Focus

You will primarily work with patients facing terminal illnesses and their families, providing mental health support that addresses grief, anxiety, and adjustment to life transitions. Your role is crucial in creating a nurturing space where patients can express their feelings, cope with their situations, and find peace during sensitive times.

Qualifications

• Education: Master’s degree in Social Work from an accredited institution.
• Licensure: Current and valid LCSW licensure in the state of Illinois.
• Experience: A minimum of two years post-licensure experience, preferably within hospice, palliative care, or a mental health setting.
• Knowledge and Skills: Comprehensive understanding of mental health disorders, therapeutic modalities, and trauma-informed care approaches.
• Proficiency in conducting assessments and formulating treatment plans tailored to individual needs.
• Strong crisis intervention skills with the ability to navigate high-stress situations effectively.
• Exceptional interpersonal and communication skills, vital for forming alliances and gaining trust from diverse patients and families.
• Experience with electronic health records (EHR) systems is strongly preferred.

Preferred Experience

• Experience in hospice, palliative care, or similar environments.
• Demonstrated capacity to work collaboratively in multidisciplinary teams.
• Previous involvement in community outreach and psychoeducation initiatives.

Compensation and Benefits

The salary for this role ranges between USD 80,000 and 100,000 per year, commensurate with experience and qualifications.
