Status: Full-time,permanent position (40 hours)

Standard working hours: 8am to 5pm EST, M-F
Location: Remote opportunities available in the following states: Virginia, North Carolina, Alabama, Delaware, Florida, Georgia, Idaho, Indiana, Kansas, Louisiana, Maine, Maryland, Minnesota, Nebraska, Nevada, New Hampshire, North Carolina, North Dakota, Ohio, Oklahoma, Pennsylvania, South Carolina, South Dakota, Tennessee, Texas, Utah, Virginia, Washington (state), West Virginia, Wisconsin, Wyoming. With occasional onsite meetings in Virginia Beach or Richmond, VA
Overview
  • Lead and develop a team of Care Managers supporting members in Nursing Facilities and Skilled Nursing Facilities.
  • Supervise 2 direct reports and provide oversight to 23 indirect team members.
  • Drive process improvement initiatives to enhance team performance and operational efficiency.
  • Monitor quality outcomes and support closure of member care gaps.
  • Prepare and present operational updates, metrics, and deliverables to leadership.
  • Collaborate with providers, facility staff, and internal teams to improve member outcomes.

Education
  • Bachelor's Degree REQUIRED

Certification/Licensure
  • One of the following is REQUIRED :
    • Registered Nurse License (RN) - Nursing License - Compact/Multi-State License or
    • Licensed Clinical Social Worker (LCSW) - State license - North Carolina Social Work Certification and Licensure Board (NCSWCLB) or
    • Licensed Clinical Mental Health Counselor (LCMHC) - State License - North Carolina Board of Licensed Clinical Mental Health Counselors (NCBLCMHC) or
    • Licensed Professional Counselor (LPC) - State license - Virginia Department of Health Professionals (VADHP) or
    • Licensed Professional Counselor (LPC) - State license - North Carolina Board of Licensed Clinical Mental Health Counselors (NCBLCMHC)

Experience
  • 3 years of case management experience REQUIRED
  • 1 year of leadership experience REQUIRED
  • With extensive Nursing Facility or Long Term Care Experience preferred
  • Has knowledge of Long-Term Services and Supports (LTSS) / Home and Community-Based Services (HCBS) or Medicaid requirements
