This position is primarily remote, providing flexibility and convenience for our team members. However, it functions within a climate-controlled hospice center where interaction with various healthcare professionals is vital. Employees will be required to adapt to a dynamic office environment, maintaining effective communication and collaboration within the team.

• Conduct thorough assessments, diagnostic evaluations when necessary, and ongoing evaluations of patients' mental health status utilizing established tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM).
• Develop individualized treatment plans that reflect cultural sensitivity and meet the unique needs of our patient population.
• Continuously monitor patient progress and implement targeted interventions and strategies to support diagnosed mental health conditions effectively.
• Accurately prepare and maintain required treatment notes, reports, and clinical documentation within established timeframes, utilizing current technology and adhering to regulatory standards.
• Collaborate with patients, families, and interdisciplinary teams to create comprehensive treatment plans that align with patient goals and desired outcomes.
• Facilitate referrals and maintain effective communication channels between patients and treatment providers, coordinating care to optimize health outcomes.
• Offer mentorship and education for interns, students, and volunteers while also engaging in peer review and clinical collaboration efforts to enhance team development.
• Assist in quality improvement initiatives within the department as directed by clinical supervisors.

The primary focus of this role is to support individuals and families impacted by cancer diagnoses. Our ideal candidate will understand the complexities involved in cancer care and will approach each case with sensitivity, aiming to improve patients’ emotional, psychological, and social well-being.

• A Master’s degree in Social Work from an accredited institution.
• A current and valid State License as a Licensed Clinical Social Worker (LCSW).
• At least two years of supervised clinical experience providing therapy and support within mental health settings.

Experience working in hospice, oncology care, or a related field targeting the emotional and mental health needs of patients facing serious illnesses is highly desirable. Familiarity with evidence-based therapeutic practices tailored to cancer care would be an asset.
