This position is fully remote, allowing you the flexibility to provide support while working from the comfort of your own home. You will engage with patients and families via virtual sessions, utilizing technology to offer therapeutic interventions. Although you will work primarily from home, you will be part of a supportive team of healthcare professionals, including nurses, dietitians, and spiritual counselors, who work together to ensure the best outcomes for our patients.

• Conduct thorough psychosocial assessments to identify patients’ emotional and social needs related to their terminal illnesses.
• Provide individual, family, and group therapy sessions using evidence-based methodologies to address coping mechanisms and resource navigation.
• Develop and implement individualized treatment plans tailored to the unique needs and goals of each patient.
• Assist patients and family members with grief and loss counseling, addressing anticipatory grief and bereavement.
• Collaborate with healthcare teams, including physicians and psychologists, to coordinate and enhance the care plan.
• Offer crisis intervention and support during acute emotional distress, ensuring patient safety and emotional stability.
• Facilitate group therapy sessions and educational workshops for families on coping strategies and available resources.
• Maintain complete and timely documentation of all patient interactions, adhering to ethical and legal standards in social work practice.
• Stay informed about community resources and assist patients in accessing appropriate support services.

You will primarily work with patients in hospice care who are facing serious, often terminal diagnoses including cancer, heart disease, and neurological disorders. The care you provide will target emotional, psychological, and social challenges, ensuring that both patients and their families experience support that enhances their quality of life and dignity during end-of-life care.

• A Master's degree in Social Work from an accredited institution.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Nevada.
• At least two years of direct clinical experience in a mental health or outpatient setting, with a preference for hospice or palliative care experience.
• Strong knowledge of evidence-based therapeutic modalities, specifically those related to grief counseling and trauma-informed care.
• Exceptional interpersonal, communication, and assessment skills to effectively connect with patients and their families.

• Experience working with bereaved individuals and their families within a hospice framework.
• Familiarity with electronic health records (EHR) systems for managing patient documentation efficiently.
• Demonstrated ability to work collaboratively within interdisciplinary teams, facilitating patient care across various domains.
