The Remote Oncology/Hematology Registered Nurse provides telephonic and remote care coordination services, assessing patient needs and collaborating with qualified staff to ensure appropriate, timely care. This role involves coordinating with the broader care team, documenting all care elements in the patient's medical record, accurately identifying high-risk conditions, proactively supporting patients through the care coordination, patient education, and directing additional care to appropriate resources as necessary. • Conduct telephonic assessments of patients, determining the necessary level of care and facilitating outpatient and/or telehealth visits when symptoms can be managed in that setting.
  • Educate patients on symptom management, medication side effects, and treatment plans, providing clear instructions and empathetic support to enhance their understanding and compliance.
  • Collaborate with other members of the healthcare team to ensure comprehensive patient care, documenting all interactions meticulously in the electronic health record (EHR) system.
  • Utilize and adapt quickly to various technology and reporting platforms to ensure efficient and accurate remote patient care and documentation.
