Key Responsibilities

• Act as a liaison between clients, their families, and outpatient service providers to ensure comprehensive, integrative care.
• Engage with insurance companies to manage pre-certification and utilization processes, advocating for our clients’ needs and entitlements.
• Participate actively in discharge and aftercare planning, ensuring continuity of care for clients transitioning from intensive treatment services.
• Offer therapeutic meal support, demonstrating and promoting a healthy relationship with food as part of holistic client care.
• Provide safe, empathetic, and culturally affirming care, tailoring strategies to meet each client's unique needs and acknowledging their intersecting identities.

Patient Population / Client Focus

Your work will primarily focus on supporting clients in a detox center setting, often dealing with challenges related to substance use and mental health disorders. You will engage with individuals from diverse backgrounds, providing tailored support that respects and integrates their unique circumstances and identities to foster personal healing and recovery.

Qualifications

• Master's degree in Social Work (MSW) from an accredited institution is required.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Texas is mandatory.
• Experience working with older adults and a strong understanding of their specific mental health challenges is essential.
• Demonstrated strong clinical and diagnostic skills to assess client needs and implement effective treatment strategies.
• Exceptional communication and interpersonal skills, enabling effective collaboration with clients, families, and other healthcare providers.

Preferred Experience

• Prior experience in a detox or rehab center is a significant advantage.
