Registered Nurse (Home Health) – Full-Time/Part-Time/Contingent Positions Available

Location: Michigan

About Us

Bilquis Home Health Care, LLC is a compassionate, culturally sensitive home health agency dedicated to delivering high-quality care in the comfort of our patients' homes. We are seeking a skilled and dedicated Registered Nurse (RN) to join our team and provide exceptional care to our home health patients throughout Michigan.

Position Summary

The Home Health RN is responsible for delivering skilled nursing care in accordance with each patient’s individualized plan of care. Duties include conducting assessments, coordinating care, educating patients and families, and collaborating with a multidisciplinary team.

Key Responsibilities

  • Conduct comprehensive patient assessments (including OASIS assessments) at admission, recertification, and discharge.
  • Develop, implement, and revise individualized care plans.
  • Provide direct skilled nursing care and education to patients and caregivers.
  • Monitor and evaluate patient progress and report changes to the physician and Clinical Manager.
  • Supervise and evaluate care provided by Home Health Aides and LPNs.
  • Coordinate services with physical therapists, social workers, and other care team members.
  • Maintain timely and accurate documentation and clinical notes (submitted within 48 hours).
  • Participate in case conferences, staff meetings, and quality improvement activities.

Qualifications

  • Graduate of an accredited School of Nursing.
  • Active Registered Nurse license in the State of Michigan.
  • At least 1 year of home health or acute care experience preferred.
  • Experience with Medicare, Medicaid, and OASIS documentation.
  • Strong organizational and communication skills.
  • Must have a valid driver’s license, reliable transportation, and auto insurance.
  • CPR certification (in-person training only; online not accepted).
  • Must pass a background check.

Work Conditions

  • Frequent driving to patients' homes required.
  • Ability to lift, turn, and assist patients in various physical conditions.
  • Must be able to conduct physical assessments and communicate clearly in English (both verbal and written).
  • Bilingual candidates are a plus!
