• Provide remote therapy through an 8-week evidence-based program designed to help clients cultivate practical skills and healthy habits.
• Monitor and document participant progress meticulously, offering follow-up communications and personalized care coordination recommendations.
• Collaborate as an integral part of a multidisciplinary care team, helping participants access necessary resources and navigate consent and authorization processes.
• Identify potential high-risk situations with clients and take action according to established protocols to ensure their safety and well-being.
• Engage in ongoing professional development by attending scheduled meetings with clinical supervisors, monthly team discussions, and organizational Town Halls.
• Uphold the highest standards of confidentiality in compliance with HIPAA regulations, ensuring all documentation and training are up to date.
