Thrive Compassionate Counseling is seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity. You will communicate and collaborate with a diverse team of rehabilitation professionals, leveraging advanced technologies and methodologies to enhance patient recovery.

• Conduct comprehensive psychosocial assessments through patient interviews, observations, and detailed medical record reviews. These evaluations help ascertain the emotional, social, and environmental factors that may be impacting each patient’s recovery process.
• Provide individualized counseling and emotional support to patients and their families, particularly in addressing adjustment issues, grief, and coping strategies.
• Work collaboratively with our interdisciplinary rehabilitation team to develop tailored treatment plans that incorporate identified psychosocial needs and rehabilitation goals.
• Assist patients in navigating community resources, such as integration into support groups, vocational rehabilitation, and housing assistance, to foster continued personal growth outside the clinical setting.
• Educate patients and their families about available community resources and facilitate post-rehabilitation services that support holistic recovery.
• Engage actively in discharge planning, ensuring smooth transitions for patients as they return home or move to alternative care settings, reflecting continuity of care.

In this role, you will be pivotal in supporting a diverse range of patients, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Your expertise will guide patients through emotional and strategic processes to regain autonomy and enhance functional skills critical for daily living.

• A Master’s degree in Social Work from an accredited institution is required.
• Current active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• Proficient understanding of adjustment issues, grief response, and coping mechanisms as they relate to rehabilitation.
• Compelling communication and interpersonal skills are essential for successful interactions with patients, families, and fellow team members.

• Prior experience in a rehabilitation or medical setting is preferred, reflecting familiarity with the complexities involved in recovery.
