Job description

Position Overview

Imaginative Insights Counseling Solutions seeks a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team. As a full-time clinical social worker in a remote capacity, you will play a vital role in providing support and empowerment to individuals facing significant life challenges. Key Responsibilities
  • Conduct comprehensive psychosocial assessments through structured interviews and thorough review of patient histories and medical records. Evaluate emotional, social, and environmental factors influencing patient recovery.
  • Deliver individual and family counseling sessions, helping patients and their families address adjustment issues, emotional challenges, and coping strategies post-injury or illness.
  • Collaborate with the rehabilitation team to create tailored treatment plans that align with each patient’s unique psychosocial needs.
  • Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance programs.
  • Educate patients and families about available community resources and facilitate their access to necessary post-rehabilitation services.
  • Contribute to discharge planning by coordinating transitions to home or other forms of care while ensuring continuity of care and support.
Patient Population / Client Focus

You will work with a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Your role is pivotal in addressing the psychosocial aspects of recovery, helping patients to reintegrate effectively into their communities and regain a sense of normalcy in their lives.

Qualifications
  • Master's degree in Social Work from an accredited institution.
  • Active state license as a Licensed Clinical Social Worker (LCSW) in New York.
  • Strong interpersonal and communication skills to foster relationships with patients, families, and colleagues.
  • Understanding of community resources and post-rehabilitation services available for patients.

Compensation and Benefits
