This position is remote, allowing flexibility while still providing essential support to our patients and their families. You will work closely with physical therapists, occupational therapists, nurses, and medical providers, fostering an interdisciplinary approach to patient care. While this role is primarily remote, occasional team meetings and collaborative case discussions may require your participation via telehealth platforms or video conferencing tools.

• Conduct comprehensive psychosocial assessments through structured interviews and thorough review of patient histories and medical records. Evaluate emotional, social, and environmental factors influencing patient recovery.
• Deliver individual and family counseling sessions, helping patients and their families address adjustment issues, emotional challenges, and coping strategies post-injury or illness.
• Collaborate with the rehabilitation team to create tailored treatment plans that align with each patient’s unique psychosocial needs.
• Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance programs.
• Educate patients and families about available community resources and facilitate their access to necessary post-rehabilitation services.
• Contribute to discharge planning by coordinating transitions to home or other forms of care while ensuring continuity of care and support.

You will work with a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Your role is pivotal in addressing the psychosocial aspects of recovery, helping patients to reintegrate effectively into their communities and regain a sense of normalcy in their lives.

• Master's degree in Social Work from an accredited institution.
• Active state license as a Licensed Clinical Social Worker (LCSW) in New York.
• Strong interpersonal and communication skills to foster relationships with patients, families, and colleagues.
• Understanding of community resources and post-rehabilitation services available for patients.
• Demonstrated ability to work collaboratively in an interdisciplinary team environment.

• Experience in a rehabilitation or medical setting is highly preferred, as it equips you with the skills necessary to navigate patients’ complex needs.
• Proficiency in managing cases with a focus on adjustment issues, grief, and coping mechanisms.
• Familiarity with telehealth services and digital patient management systems.

The salary for this full-time role ranges from USD 75,000 to 100,000 per year, commensurate with experience and expertise. Additionally, a sign-on bonus is available to enhance your compensation package.
