• Conduct comprehensive assessments of clients' psychosocial and emotional needs to develop tailored care plans.
• Provide individual and family counseling to seniors, addressing issues related to mental health, life transitions, and emotional resilience.
• Work collaboratively with healthcare professionals to coordinate services that support clients’ overall wellness, adjusting care strategies based on their evolving needs.
• Engage in case management activities that ensure seamless access to community resources, assistance programs, and support services.
• Document all client interactions, assessments, and progress in the electronic health record system, ensuring compliance with confidentiality and legal standards.
• Facilitate conflict resolution among family members involved in client care and advocate for the clients' rights in accessing essential services.
• Regularly evaluate living conditions and safety of clients, addressing any concerns that may arise during home visits or discussions.
• Stay abreast of best practices, new policies, and regulations in social work to maintain ethical standards and enhance service delivery.

As a Clinical Social Worker, you will have the opportunity to engage directly with these individuals and their families, providing guidance, counseling, and support that enhances their quality of life.

• Master's degree in Social Work from an accredited institution.
• Current and valid licensure as a Clinical Social Worker (LCSW-C) in the state of Maryland.
• Minimum of two years of professional experience in a social work capacity, ideally with a focus on geriatric populations.
• Deep understanding of aging-related issues, social work principles, and community service resources.
• Exceptional communication, interpersonal, and problem-solving skills, with a strong ability to create rapport with clients and families.
• Proficient in utilizing electronic health record (EHR) systems to manage client information.

• Experience working in a community health center or a similar setting focused on geriatric care.

• Background in crisis intervention and conflict resolution strategies.
