• Conduct thorough psychosocial assessments of seniors to evaluate their emotional, mental, and social needs, providing an empathetic listening ear and detailed insights.
• Develop and implement personalized care plans in collaboration with clients and their families, ensuring alignment with individual goals and preferences.
• Provide counseling support to assist seniors in navigating life transitions, including bereavement, health challenges, and changing living situations.
• Act as a strong advocate for clients, facilitating access to community resources and services that promote their health and well-being.
• Work closely with interdisciplinary teams, including healthcare professionals and caregivers, to ensure coordinated and comprehensive care.
• Regularly monitor clients through virtual consultations, addressing any emerging concerns regarding their living conditions and safety.
• Facilitate communication among family members involved in care, helping to mediate conflicts and fostering a supportive family environment.
• Educate clients and families about available community support services, aiding in their social engagement and empowering them to access assistance.
• Maintain accurate and up-to-date documentation of client interactions, assessments, and progress notes, while ensuring compliance with ethical standards.
• Stay informed on current practices and policies in social work, integrating new approaches to enhance service delivery.

In this role, you will primarily focus on supporting seniors within the community, helping them to navigate the complexities of aging. Many clients may face challenges related to physical health conditions, social isolation, emotional distress, or family dynamics. Your interventions will be critical in helping them achieve stability, resilience, and enhanced quality of life.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years' experience in social work, particularly within community health or geriatric settings.
• In-depth knowledge of relevant social work principles, aging issues, and available community resources.
• Strong assessment, counseling, and problem-solving skills, coupled with a compassionate approach to client interactions.
• Exceptional communication skills, enabling effective collaboration with various interdisciplinary teams.
• Proficient in electronic health records (EHR) systems and technology-based client management tools.

• Experience working with seniors and understanding the diverse challenges they face.
• Familiarity with telehealth practices and remote service delivery.
• Strong history of advocacy for client rights and access to resources.
