Key Responsibilities
  • Collaborate with healthcare team members to develop comprehensive treatment plans, including discharge plans grounded in psychosocial assessments and established social work interventions.
  • Engage with leaders of community resource groups to represent our organization, actively participating in meetings and committees focused on addressing the social needs and concerns of our patients.
  • Provide individual and group counseling sessions, applying crisis intervention strategies for patients facing emotional and behavioral challenges, including thorough assessments and effective treatment modalities.
  • Maintain ongoing communication with physicians and other healthcare providers, effectively reporting patient progress, unique needs, concerns, as well as anticipating potential issues that may arise.
  • Document clinical and progress notes accurately and in a timely manner, adhering to departmental policies to ensure compliance and facilitate continuity of care.

Qualifications
  • A Masterโ€™s degree in Social Work (MSW) and current licensure as a Clinical Social Worker (LCSW) is required.

  • Strong ability to build professional relationships with colleagues, patients, and multidisciplinary teams, fostering a collaborative environment.
