The Divine Psychological Wellness Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team on a full-time basis. Key Responsibilities
  • Manage a diverse caseload of clients seeking detox services, providing tailored clinical social work services that meet their unique needs.
  • Conduct comprehensive assessments of clients and their families to identify social, emotional, and environmental factors contributing to their challenges.
  • Facilitate care coordination by collaborating with multidisciplinary teams, including psychiatrists, medical staff, and case managers to develop and implement holistic treatment plans.
  • Assist in the discharge planning process by evaluating available resources, making referrals to community services, and providing ongoing support to ensure successful transitions.
  • Document patient interactions accurately in our EHR system, maintaining compliance with healthcare regulations and ensuring accountability in all professional practices.

Patient Population / Client Focus

Qualifications
  • A Masterโ€™s degree in Social Work (MSW) from an accredited institution is required.
  • Current license as a Licensed Clinical Social Worker (LCSW) in California.
  • Minimum of 2 years of experience in social work, preferably in an inpatient hospital or community healthcare setting.
