• Conduct comprehensive mental health assessments for hospice patients, thoughtfully considering their medical history, psychological state, and emotional needs.
• Develop and implement individualized treatment plans aimed at addressing mental health challenges while promoting emotional stability and peace during end-of-life transitions.
• Provide evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT) and supportive therapy tailored to the unique circumstances facing our patients.
• Facilitate individual counseling sessions via telehealth platforms to support patients and their families in coping with grief, loss, and existential concerns.
• Collaborate closely with a team of healthcare professionals, including registered nurses, physicians, and chaplains, to create a unified approach to patient care.
• Offer crisis intervention services when needed, applying de-escalation techniques to handle sensitive situations with care and professionalism.
• Engage with families to provide them with psychoeducation about mental health resources and processes, helping them navigate the complexities of hospice care.
• Maintain meticulous records of patient assessments, treatment plans, and progress notes, adhering to legal and ethical standards in documentation.
• Participate in regular team meetings and continuing education to enhance collaborative practices and improve care outcomes.

Your work will directly impact individuals in hospice care, including patients with terminal illnesses as well as their family members who are navigating emotional turmoil, anticipatory grief, and the various challenges associated with end-of-life preparations. Supporting this client population requires sensitivity, understanding, and a deep commitment to enhancing their quality of life during challenging times.

• Education: A Master’s degree in Social Work from an accredited institution.
• Licensure: Current and valid LCSW license in the state of Nevada.
• Experience: At least two years of post-licensure clinical experience, particularly within mental health or hospice settings.
• Proficient understanding of mental health diagnoses, treatment modalities, and trauma-informed care principles.
• High proficiency in assessment and evaluation of mental health needs, with the capability to develop suitable treatment approaches.
• Experience with therapeutic interventions, particularly in a crisis-oriented environment.
• Strong communication skills, essential for building rapport with patients from diverse backgrounds.
• Familiarity with electronic health records (EHR) systems and documentation practices.

• Previous experience providing social work support in hospice or end-of-life care settings.
• Experience in family therapy or group counseling would be an asset.
• Knowledge of community resources available for patients and families dealing with grief and terminal illness.
