Key Responsibilities
  • Conduct comprehensive assessments of seniors' psychosocial needs, critically evaluating emotional, mental, and social aspects that affect their overall well-being.
  • Collaborate closely with clients and their families to develop personalized care plans that address their specific goals and preferences, emphasizing a client-centered approach.
  • Provide counseling and emotional support, guiding seniors and their families through significant life transitions, such as loss or relocation.
  • Advocate effectively for client rights, ensuring they have access to necessary community resources, services, and support systems.
  • Coordinate with interdisciplinary teams—comprised of nurses, therapists, and other healthcare professionals—to enhance care delivery tailored to the individual needs of each senior.
  • Regularly visit clients to monitor their well-being, evaluate living conditions, and address any safety or health-related concerns that arise.
  • Facilitate discussions and resolve conflicts among family members involved in the client's care, ensuring a harmonious and productive care environment.
  • Educate seniors and their families about available community resources, support groups, and programs designed to enhance social engagement and overall life satisfaction.
  • Maintain accurate documentation of client assessments, care plans, progress notes, and notable interactions to ensure compliance and provide continuity of care.
  • Stay informed about evolving social work practices, state regulations, and ethical standards to maintain high-quality professional standards.
Patient Population / Client Focus

The primary focus of your work will be elderly clients, often facing various psychosocial issues that impact their daily lives. You will work with seniors transitioning through critical life stages, experiencing health challenges, or facing isolation. Your compassionate approach and professional interventions will significantly impact their mental and emotional health.

Qualifications
  • A Master's degree in Social Work from an accredited institution is required.
  • A current and valid state licensure as a Licensed Master Social Worker (LMSW).
  • A minimum of two years of relevant experience, preferably within community centers or settings focused on geriatric care.
  • In-depth knowledge of social work principles, aging issues, and available resources in the Dallas community.
  • Strong assessment, counseling, and analytical skills.
  • Exceptional ability to empathize and build rapport with seniors and their families.
