Key Responsibilities
  • Conduct thorough assessments of patients to tailor therapy to their specific needs, utilizing evidence-based practices.
  • Maintain open lines of communication with supervisors, direct care staff, family members, and guardians to provide updates on client progress.
  • Schedule appointments, including home visits, and monitor ongoing treatment to adapt plans as necessary.
  • Coordinate essential home visits for in-home observations and data collection to keep treatment plans aligned with client goals.
  • Ensure the effective implementation of program plans and document all procedures accurately for compliance and reporting purposes.
  • Engage actively in Interdisciplinary Team Meetings to discuss care strategies and collaborate on patient treatment.
  • Create and complete detailed monthly and quarterly progress notes documenting client advances and barriers.
  • Design and facilitate training sessions to ensure staff competence in program implementation and documentation standards.
  • Conduct follow-up interviews and home assessments, providing continuous support to clients as needed.
  • Serve as a liaison within the mental health community, fostering collaboration and resource sharing for developmental goals.
  • Support in the creation of training materials to enhance program initiatives.
Patient Population / Client Focus

In this role, you will work with a diverse clientele, including both adults and youth, addressing various mental health challenges. Your expertise may be particularly beneficial for clients dealing with trauma, substance misuse, or family dynamics, allowing you to employ specialized therapeutic techniques and interventions that are tailored to individual cases. Qualifications
