Key Responsibilities

• Conduct thorough psychosocial assessments through interviews, observation, and review of medical records to evaluate the complex emotional, social, and environmental factors affecting patients' recovery.
• Provide compassionate counseling and support services to patients and their families, helping them address emotional and adjustment issues related to their health conditions.
• Collaborate closely with a multidisciplinary rehabilitation team to create and implement individualized treatment plans that consider patients' psychosocial needs.
• Guide patients in accessing essential community resources, such as support groups, vocational rehabilitation programs, or housing assistance, enhancing their rehabilitation and recovery.
• Deliver educational resources and ongoing support to patients and their families regarding available community services and post-rehabilitation options to ensure a smooth transition back into daily life.
• Participate actively in discharge planning, facilitating transitions to home care or other environments as necessary, ensuring continuity of care.

Patient Population / Client Focus

The LCSW at the American Counseling Center will work with an array of patients, including those recovering from strokes, spinal cord injuries, and other orthopedic and neurological disorders. Qualifications

• A valid LCSW license issued by the state of Texas is required.
