As the Licensed Clinical Social Worker, your primary responsibilities will include:

• Conducting comprehensive psychosocial assessments through techniques such as clinical interviews, patient observations, and the analysis of medical records to identify emotional and social factors affecting patients' recovery.
• Providing individual and family counseling aimed at addressing adjustment issues, grief, and emotional well-being throughout the rehabilitation process.
• Collaborating closely with an interdisciplinary rehabilitation team to formulate individualized treatment plans that integrate psychosocial needs and therapeutic objectives.
• Assisting patients in obtaining access to community resources, such as support groups, vocational rehabilitation services, and housing assistance.
• Educating patients and families on available community resources and post-rehabilitation services to facilitate smooth transitions back to everyday life.
• Participating in discharge planning and facilitating effective transitions to home or alternative care services in a timely and supportive manner.

The clientele at our detox center consists of individuals facing various challenges related to substance use disorders. These patients may require comprehensive support, including coping strategies for managing withdrawal symptoms, understanding interrelated mental health issues, and navigating adjustment issues in recovery. Your role will be instrumental in guiding them toward a successful recovery journey focused on restoring functional independence and ensuring a smooth reintegration into society.

The ideal candidate will possess the following qualifications:

• A master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong communication and interpersonal skills, enabling you to connect effectively with patients and team members.
• A firm understanding of psychological adjustment issues, grief, and coping mechanisms, essential for effective patient support.

While not mandatory, the following experiences are preferred:

• Previous work in a rehabilitation or medical setting, providing social work services to diverse patient populations.
• Experience with trauma-informed care practices, chronic illness management, and integrated mental health support.
• Familiarity with electronic health record (EHR) systems used for managing patient care documentation and coordinating services.
