Position Overview

Hospice of South Texas is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to provide direct patient care in a home-based setting. Working under the supervision of a Registered Nurse, the LVN plays a critical role in supporting patients and families through end-of-life care while collaborating closely with the interdisciplinary team.

Key Responsibilities
  • Deliver skilled nursing care in accordance with the plan of care (POC) under RN supervision
  • Support the RN in implementing and coordinating patient care needs
  • Assist patients and families with education on self-care and disease management
  • Document all clinical care, interventions, and patient responses accurately and timely
  • Communicate changes in patient condition to the RN and care team
  • Assist with procedures and specialized care related to end-of-life support
  • Prepare and maintain equipment and supplies using proper aseptic techniques
  • Ensure care is delivered in compliance with organizational policies and regulatory standards
  • Promote the mission, values, and quality standards of Hospice of South Texas
  • Participate in interdisciplinary collaboration to ensure continuity of care
  • Perform other duties as assigned
Qualifications

Education & Licensure

  • Graduate of an accredited vocational nursing program
  • Current Licensed Vocational Nurse (LVN) license in the State of Texas
  • Valid Texas Driver's License

Experience

  • Minimum of 2 years of clinical nursing experience
  • Hospice or home health experience preferred

Skills & Abilities

  • Strong clinical nursing skills aligned with accepted standards of practice
  • Excellent communication and interpersonal skills
  • Ability to respond effectively in emotionally sensitive situations
  • Strong documentation and organizational skills

Other Requirements

  • Reliable transportation and valid auto liability insurance
  • Proof of Hepatitis B vaccination or declination
Work Environment & Physical Requirements
  • Provide care in patients' homes with varying conditions
  • Frequent standing, walking, bending, and lifting (up to 50 lbs.)
  • Exposure to bloodborne pathogens and infectious diseases
  • Ability to work flexible schedules, including on-call or PRN emergency situations
  • Local travel required
