Responsibilities
If you're someone who believes that real change happens in real life - not just in an office - this role was built for you. As a Family Centered Treatment (FCT) Therapist at Health Connect America, you'll provide impactful, evidence-based services where they matter most: in the homes and communities of the children and families you serve.
What You'll Do
As an FCT Therapist, you'll provide high-quality psychotherapeutic services to children, adolescents, and families using the Family Centered Treatment model. Your work will be hands-on, relationship-driven, and grounded in clinical best practices. Day to day, you can expect to:
  • Deliver Family Centered Treatment services in home and community settings, meeting families where they are
  • Guide children and families through the four phases of the FCT model to support lasting change
  • Develop and maintain individualized Service Plans in collaboration with the Treatment Team
  • Conduct assessments and provide therapeutic interventions and psychoeducation aligned with treatment goals
  • Coordinate care with medical, behavioral health, school, and community partners to ensure integrated support
  • Support families in setting goals, building skills, and accessing community resources
  • Develop discharge and aftercare plans to promote long-term stability and success
  • Maintain timely, accurate clinical documentation in the Electronic Health Record and participate in quality assurance activities
What to Expect
This is community-based work, requiring a flexible schedule that meets the needs of the families you serve. Services are delivered in-home and in the community, often outside of traditional business hours.
This role also includes 24/7 on-call coverage for your assigned caseload, providing crisis support when it matters most. While urgent situations arise, they are not the norm - but your availability plays a critical role in maintaining stability and trust with the families you support.
Your Path to FCT Certification
FCT Certification is required for this role and must be completed within one year of employment to continue providing services. Health Connect America supports your success through structured checkoffs, required trainings, and an e-learning platform that tracks your progress.
Completing certification is more than a requirement - it's a professional milestone that strengthens your clinical skillset and positions you as a specialist in delivering evidence-based family therapy.

Qualifications
We welcome candidates who bring a genuine commitment to children and families and meet the following qualifications:
  • Minimum of a Master's Degree in Counseling, Social Work, or a related field
  • Must currently hold a provisional/associate license (e.g., LCSWA, LCMHCA, LMFTA)
  • Must meet qualifications as a Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis
  • Current FCT Certification is a plus, though not required - we'll help you get there!
