Job Description: The primary purpose of this position is to be a member of a patient's care team in order to assist in planning, developing and coordinating patient care in accordance with applicable federal, state and local standards, guidelines, regulations to assure that the highest degree of quality care can be maintained at all times.
Job Duties and Responsibilities:
  • Meets with administration, medical and nursing staff, as well as other related departments in planning services-to include patients and family
  • Partners with the patient, family members, patient's care team and Social , health and community agencies to insure a smooth transitions upon discharge from the Center
  • Ensures that all documentation is informative, descriptive and follows the Center's policy and procedures.
  • Assists with the admission and discharge process.
  • Attends care plan meeting to provide insight and professional guidance to patients needs

Experience and Specific Requirements:
  • A minimum one year case management experience
  • Must possess leadership ability and the willingness to work harmoniously with other personnel
  • Must possess good judgement and have the ability to maintain highly confidential information
  • Must be knowledgeable of HIPAA compliance and requirements
  • Must have the ability and to plan, organize, develop, implement and interpret the programs, goals and objectives of the department as it relates to the Company
  • Must be able to pass a pre-employment background check/drug screen
  • Active Fingerprint card
