At Duke Home Health, you are able to meet patients where they are. By working as a home health nurse, you help keep patients out of the hospital and in their homes where they are most comfortable. Schedule:
  • Monday-Friday - 8 hour days

How you will make an impact
  • Deliver and document professional nursing care in home settings using the nursing process in accordance with North Carolina regulations and agency standards.
  • Conduct comprehensive assessments, develop individualized care plans, and collaborate with interdisciplinary teams to deliver effective care.
  • Educate patients and families on disease processes, medications, and care plans to support outcomes and reduce hospital readmissions.
  • Coordinate care including case management responsibilities, discharge planning, and OASIS assessments tied to reimbursement and quality outcomes.
  • Ensure compliance with regulatory, safety, and quality standards while maintaining accurate and timely documentation.
What you will need
  • Associate Degree or Diploma in Nursing required; BSN encouraged after two years of service.
  • Current RN license in North Carolina or compact state.
  • CPR certification (American Heart Association).
  • Valid driver's license and current automobile insurance coverage.
  • 1-2 years of med-surgical, critical care, or home health experience preferred.
  • Knowledge of Medicare and Medicaid regulations, nursing scope of practice, and infection control standards.
  • Strong assessment, critical thinking, communication, and documentation skills with the ability to work independently in home settings.
