A premier inpatient rehabilitation hospital serving the Mid-Ohio Valley region, providing comprehensive medical rehabilitation for patients recovering from stroke, brain injury, spinal cord injury, orthopedic conditions, and other complex diagnoses. This 50-bed facility maintains a strong daily census and is known for delivering high-quality, compassionate, outcomes-driven patient care.

Job Details
  • Provide onsite leadership and oversight of the hospital's Case Management department
  • Directly manage a team of 4 full-time Case Managers, 1 full-time Case Management Assistant, and 2 PRN Case Managers (mix of RNs and Social Workers)
  • Step in to personally cover patient caseloads as needed to support the team during high-volume periods or staffing gaps
  • Oversee utilization review and discharge planning processes across a 50-bed inpatient rehab unit with an average daily census of ~49 patients
  • Ensure timely, effective discharge planning given average patient length of stay of 7–14 daysPartner closely with the hospital CEO and Regional Director of Case Management on departmental strategy and performance
  • Ensure department compliance with regulatory, licensure, and clinical documentation standards
  • Support staff development, coaching, and performance management for Case Managers and Case Management Assistant
  • Pursue CCM or ACM certification within first year if not already obtained, with employer support
