Job description

Our client is a trusted skilled nursing and long-term care facility serving the residents of Ordway, Colorado and the surrounding rural communities . POSITION OVERVIEW

We are seeking a dedicated and licensed Physical Therapist Assistant (PTA) to join our

rehabilitation team. Reporting to the Director of Rehabilitation, the PTA will deliver individualized

physical therapy services to long-term care residents and skilled nursing patients, supporting

functional recovery and quality of life under the supervision of a licensed Physical Therapist.

KEY RESPONSIBILITIES

Implement individualized physical therapy treatment plans as established and directed by

the supervising Physical Therapist

Deliver therapeutic interventions including therapeutic exercise, gait training, balance

activities, transfer training, and modality application

Monitor and document patient progress, functional status, and response to treatment in

compliance with state, federal, and facility standards

Communicate regularly with the supervising PT, interdisciplinary team, and nursing staff

regarding resident goals and progress

Educate patients, families, and caregivers on home exercise programs, fall prevention, and

safe mobility techniques

Participate in care planning meetings, utilization reviews, and MDS documentation as

applicable

Maintain timely, accurate clinical documentation in compliance with Medicare/Medicaid and

facility requirements

Uphold resident rights, dignity, and confidentiality in all interactions

Assist in maintaining a clean, organized, and safe therapy gym and treatment environment

QUALIFICATIONS

Required:

Associate's degree from an accredited PTA program

Current, unrestricted PTA licensure in the State of Colorado (or ability to obtain prior to start

date)

Current CPR/BLS certification

Strong interpersonal and communication skills with a patient-first mindset

Preferred:

1+ years of experience in a skilled nursing, long-term care, or post-acute rehabilitation

setting

Familiarity with MDS/PDPM documentation frameworks

Experience with geriatric populations and fall prevention programs

Knowledge of EMR/documentation systems used in SNF environments
