This individual will serve as a key representative of the facility within the healthcare community while coordinating referrals, admissions, and resident transitions. Community Relations Liaison Responsibilities:
Develop and maintain strong relationships with hospitals, physicians, discharge planners, case managers, assisted living communities, and other referral sources
Represent the facility at networking events, healthcare meetings, and community outreach activities to promote services and increase referrals
Coordinate the admissions process from referral through resident arrival
Receive, review, and respond promptly to referrals from hospitals and community partners
Verify insurance coverage and eligibility, including Medicare, Medicaid, and managed care plans
Collaborate with discharge planners, families, and interdisciplinary team members to ensure smooth and timely admissions
Conduct facility tours for prospective residents and families
Serve as a resource for residents, families, and referral partners by providing information regarding services, programs, and admission requirements
Ensure all admission documentation is complete, accurate, and compliant with regulatory requirements
Track referral activity, admissions data, and census trends while maintaining accurate records
Participate in census development initiatives and implement strategies to increase occupancy
Provide a positive, professional, and supportive experience throughout the admission and transition process
Community Relations Liaison Qualifications:
High school diploma or equivalent required; Associate's or Bachelor's degree in Healthcare Administration, Marketing, Business, Communications, or a related field preferred
Previous experience in healthcare admissions, business development, community relations, marketing, or skilled nursing preferred
Knowledge of Medicare, Medicaid, managed care, and insurance verification processes preferred
Strong relationship-building, networking, and customer service skills
Excellent verbal and written communication abilities
Highly organized with strong attention to detail
Ability to manage multiple priorities in a fast-paced healthcare environment
Proficiency with Microsoft Office and electronic healthcare systems
