Excelcare at Wilmington is seeking a dynamic and relationship-driven Community Relations Liaison to support both facility census growth and the admissions process. This individual will serve as a key representative of the facility within the healthcare community while coordinating referrals, admissions, and resident transitions. The ideal candidate is an excellent communicator who can build strong professional relationships, provide exceptional customer service, and ensure a seamless admission experience for residents and families.

Community Relations Liaison Responsibilities:

  • Develop and maintain strong relationships with hospitals, physicians, discharge planners, case managers, assisted living communities, and other referral sources

  • Represent the facility at networking events, healthcare meetings, and community outreach activities to promote services and increase referrals

  • Coordinate the admissions process from referral through resident arrival

  • Receive, review, and respond promptly to referrals from hospitals and community partners

  • Verify insurance coverage and eligibility, including Medicare, Medicaid, and managed care plans

  • Collaborate with discharge planners, families, and interdisciplinary team members to ensure smooth and timely admissions
