• Conduct comprehensive psychosocial assessments through interviews, observations, and medical record analysis to evaluate the impact of patients' emotional and social environments on their recovery.
• Deliver individualized counseling and emotional support to patients and their families, addressing adjustment difficulties, grief, and coping strategies linked to their health conditions.
• Collaborate closely with the interdisciplinary rehabilitation team to design and implement tailored treatment plans that take into account both psychosocial and medical needs.
• Identify and facilitate access to community resources that can assist patients, such as support groups, vocational rehabilitation services, and housing assistance programs.
• Educate patients and their families about available community resources and support systems to ensure a smooth transition after rehabilitation.
• Actively participate in discharge planning meetings, ensuring that all necessary support is in place for patients returning home or transitioning to other care settings.

The LCSW role at Equis Care primarily serves patients who are facing significant life transitions due to acute or chronic health conditions. This includes individuals struggling with health-related trauma, those dealing with chronic illness, and patients transitioning to end-of-life care. • A Master’s degree in Social Work from an accredited institution, providing the requisite theoretical foundation in social work practices.

• Excellent communication and interpersonal skills, with a notable ability to engage patients and families effectively.
