) Psychiatric Rehabilitation Services Coordinator
Join our team at Allure of Moline, a leader in providing personalized care to our residents.
We are seeking a dedicated and compassionate Psychiatric Rehabilitation Services Coordinator to join our Social Services Department in East Moline, Illinois. As a Psychiatric Rehabilitation Services Coordinator, you will play a vital role in planning, organizing, implementing, evaluating, and directing the overall operation of our Social Services Department. Your primary responsibilities will include reviewing facility policies, participating in quality assurance meetings, and providing therapeutic interventions to assist residents in coping with their transition to our long-term care facility.
Essential Duties and Responsibilities:
  • Review facility policies and procedures to ensure compliance with state and federal regulations
  • Participate in reviewing and setting policies concerning resident care and quality of life
  • Develop and implement a written, interdisciplinary plan of care for each resident
  • Provide therapeutic interventions to assist residents in coping with their transition and adjustment to the facility
  • Ensure or provide support and education to residents, family members, and significant others
  • Coordinate the resident discharge planning process and make referrals for appropriate home care services

Qualifications:
  • Bachelor's degree in a human services field (including but not limited to: sociology, special education, rehabilitation counseling, or psychology)
  • Minimum of one year of supervised experience in mental health or human services is preferred

Physical Demands:
  • Required to sit, stand, bend, and walk regularly; lift and/or move up to 25 pounds
  • Visual and auditory ability sufficient for written and verbal communication
  • The noise level in the work environment is usually moderate
