Job description

Orchard Park Family Practice, P.C. is seeking a Full-Time Physician Assistant or Nurse Practitioner to join our growing team. To work collaboratively as a licensed and credential practitioner with other members of the healthcare team to provide comprehensive care to both well and ill patients.

We have been awarded recognition at the highest level by the National Committee for Quality Assurance (NCQA) for Physician Practice Connections-Patient Centered Medical Home. Our team includes five dedicated board-certified physicians and five highly trained mid-levels as well as registered nurses, and other medical professionals.

Three-five (3-5) years of experience in an acute and family practice setting is preferred.


Responsibilities:

  • Work as part of the OPFP provider staff seeing patients in face-to-face visits and abide by all the requirements of being part of the OPFP medical staff.
  • Provide high quality primary care to Orchard Park Family Practice’s patients of infant, pediatric, adolescent, adult, and geriatric groups.
  • Assesses patient needs within an acute care setting.
  • Develops, implements, evaluates, and alters the plan of care as needed
  • Utilizes critical thinking to synthesize and analyze collected data
  • Ensure high quality primary care is delivered cost-effectively, consistently.
  • Follows evidence-based standards for ordering laboratory, radiology, and other diagnostic studies.
  • Uses evidence-based resources for treatment options.
  • Orders and prescribes appropriate medications.
  • Provides care that is patient and family centered, compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease throughout the lifespan.
  • Facilitates consistent, coordinated care and clear communication among all members of the healthcare team.
  • Provide appropriate specialist referrals and follow-up recommendations when indicated.
  • Maintains effectiveness where circumstances and priorities are changing. Is flexible, resilient and change oriented.
  • Maintain electronic medical records in compliance with Orchard Park Family Practices standards with high efficiency.
  • Dedication to and accountability for optimizing antibiotic prescribing and patient safety, with a commitment to Antibiotic Stewardship.
  • Performs procedures appropriate type of services provided at the standard of care.
  • Committed to the specialty of Acute and Primary Care, with a focus on serving our patients with quick, convenient, and quality care with continued clinical and patient experience training.

  • Manage a limited, telephone-only call-coverage shared with other clinicians (weekends: every 6-7th week)
