Job Summary:
The PROMISE Clinic, an integrated behavioral health program that supports pregnant and postpartum patients as part of their prenatal care at UCHealth Anschutz, has an opening for a full-time Care Coordinator (Healthcare Entry to Intermediate Professional). This position primarily supports patients experiencing mental health symptoms by improving access to care and coordinating services through referral management, appointment scheduling, patient outreach, and assistance navigating PROMISE and other behavioral health services. The Care Coordinator also supports essential program operations, including maintaining patient and referral data, developing relationships with referring providers, and assisting with clinical data collection and reporting.
Key Responsibilities:
Serve as a primary point of contact for PROMISE patients, supporting referrals, scheduling, forms, resources, and access to services.
Manage referrals from receipt through scheduling and connection with care, including follow-up with patients and referring providers.
Provide patients with PROMISE and community resource information and schedule and reschedule appointments in Epic and conduct outreach regarding referrals, missed visits, and incomplete paperwork.
Document referral and care-coordination encounters in accordance with program and institutional requirements.
Maintain patient confidentiality and promptly route clinical questions, safety concerns, medication questions, and treatment requests to licensed clinicians.
Maintain relationships with referral sources and support outreach to referring practices and community partners, accurate referral, census, scheduling, and care-coordination data in Epic, REDCap, Excel, and other approved systems.
Track key patient and program metrics, including census, six-month postpartum status, and patients needing clinician review for discharge and collect and compile clinical and program data for grant reporting, productivity, evaluation, quality improvement, and planning.
Work with program leaders, providers, and staff to support daily operations and process improvement and provide administrative and operational support for PROMISE program activities and team functions.

The Department of Psychiatry is one of the largest units in the UCD-SOM and is comprised of 5 Divisions. The PROMISE Clinic is an integrated behavioral health program at UCHealth Anschutz that supports pregnant and postpartum patients as part of their prenatal and postpartum care. PROMISE provides timely access to behavioral health services through close collaboration among patients, obstetric providers, and mental health providers. The program focuses on identifying mental health needs, connecting patients with appropriate services and resources, coordinating referrals and follow-up, and supporting continuity of care throughout the perinatal period. Minimum Qualifications:
Entry Level:
Bachelor's degree in Psychology, Counseling, Social Work, Public Health, or related field from an accredited institution
Intermediate level:
Bachelor's degree in Psychology, Counseling, Social Work, Public Health, or related field from an accredited institution
1 year of experience healthcare systems with patient navigation, scheduling, or similar
Applicants must meet minimum qualifications at the time of hire.
Preferred Qualifications:
Spanish language proficiency
Experience with REDCap or other data management program.
Experience with and/or training in perinatal and infant mental health
Experience with crisis management or triage
Experience with program development, evaluation, and/or process improvement
Experience with EPIC scheduling functions
Experience with Excel and data entry
