Shelter Ministry Case Manager Job Description

Position:ย Shelter Ministry Case Manager

Summary

Primary responsibility is direct service case management,ย housing support and spiritual careย for guests at the New Life Center.ย Key elements involveย case management services that lower housing barriers,ย assistanceย in securing housing, collaboration with supportive services in the community, relationship building, support and accountability for guests, and helping individuals to a place of self-sufficiency.ย ย ย

Essential Functions

Essential Function 1:ย Direct Service Related

  • Perform housing assessmentsย with guestsย andย enter their assessmentย into the Community Coordinated Entry System in the HMIS database.ย 
  • Create andย follow up onย an individualizedย case managementย plan based on specific guestย needsย and desires.ย ย ย 
  • Assistย guests in lowering barriers to becomingย successfullyย housed.ย 
  • Maintain a database of local resources.ย 
  • Maintain an active caseload of 25 to 30 guests, exercisingย appropriate professionalย boundaries and workload management to avoid exceeding this range.ย 

Essential Function 2: Housing Support

  • Assistย guests in finding or applying for housing.ย 
  • Assistย guests in securing financialย assistanceย to secure housing, such as down payments and/or damage depositย assistance.ย 
  • Collaborate with community-wide supportive services to help guests remain successfully housed.ย 
  • Assistย guestsย in securingย furniture andย housewaresย through community voucher programs.ย 
  • Attendย coordinated entry andย partner agency meetings.ย 

Essential Function 3:ย Other Considerations

  • Teamwork -ย Work collaboratively with administration and other shelter staff to meet day-to-day organizationalย needs.ย ย 
  • Resource awareness and networking -ย Activelyย participateย in networking and collaboration with local serviceย providers.ย ย ย ย 
  • Data entryย - Entering and monitoring of data, case notes/logsย in relevant guest database(s).ย ย ย 
  • Customer service focusย -Treat all who come to the New Life Center seekingย assistanceย with respect and dignity, with theย ultimate goalย of helping them to a place ofย self-sufficiency.ย ย ย 
  • Meetingsย -ย As required, attend various meetings. Some travelย isย required.ย ย ย 
  • Integrity and confidentialityย -ย Maintainย a high levelย of personal integrity and confidentiality relating to shelter operations and those we serve.ย ย 
  • Participate in program development, to include basic needs, life skills, and faith (Christian) focusedย programs.ย ย ย ย 
  • Participate in a rotation of on-call staff to address program staff questions, concerns, and other programmatic needs when management or supervisors are unavailable.ย ย 
  • Ability to driveย a 15-passenger van.ย

Additional Eligibility Requirements:

  • Ability to pass a pre-employmentย drug test and background check.ย ย 
  • If in recovery, a minimum of one year of sustained recovery with active participation in a 12-step program isย required; two or more years is preferred.ย 
  • Mustย possessย a valid driverโ€™s license,ย maintainย an acceptable driving recordย in accordance withย the organizationโ€™s insurance carrier requirements, and provide proof of insurance,ย asย requiredย for the performance of job duties.ย

  • Operation of a motor vehicle, including the ability to drive a 15-passenger van,ย asย required.ย 
  • May work in varied indoor environments, including offices, meeting rooms, and common guest areas.ย 
  • Work schedule may include extended periods of computer use and documentation tasks.ย
