Valley's CORE II program provides safe, evidence-based residential treatment for adults navigating mental health challenges, substance use disorders, trauma, and other complex life barriers. As a Case Manager, you'll play a vital role in helping clients achieve meaningful progress by providing rehabilitative services, coordinating care, supporting individualized goals, and building trusted relationships that foster lasting recovery. Pay: Dependent on Case Manager certification status

  • Uncertified starts at $17.50 - certification will be obtained within the first 120 days and pay will be increased to certified rate
  • Certified range starts at $19.61 - pay is calculated based on years of certified experience

Schedule (2 options):

  • Thursday - Monday 8am-4pm (5 eight-hour shifts)

Job Summary & Deliverables

Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.

  • Provides psychosocial rehabilitative services as described by the Medicaid manual
  • Supports clients in following individualized care plans and identifying case management goals
  • Completes documentation on services provided within Valley’s standards
  • Meets productivity expectations as assigned by supervisor
  • Distributes medications as necessary
  • Transports clients to appointments as necessary
Requirements:

Licenses/Certification

  • Case Manager certification (post hire as needed - required within 120 days of start date)
  • CPR certification (post hire as needed)
  • Driving positions require a minimum age of 21 and a current driver’s license

Preferred Qualifications

  • Previous social services or behavioral health experience
  • Bachelor’s degree in Social Services, Psychology, or related field
