Registered Nurse - Hospice

ABL Hospice, LLC the Hospice agency is seeking an experienced Registered Nurses (RN) with at least 1 year of Hospice experience. By joining our team providing the highest quality experience for patients in the comfort of their own home.

Responsibilities

  • Conducts an initial assessment of patients and their families to determine their home care needs.
  • Performs a comprehensive physical examination and reviews the patient's current and past medical history.
  • Regularly reassesses the nursing requirements of the patient.
  • Initiates and adjusts the care plan based on changes in the patient's condition and needs, using health assessment data to determine nursing diagnoses.
  • Develops a care plan with goals derived from nursing diagnoses, including therapeutic, preventive, and rehabilitative nursing interventions, involving both the patient and their family in the planning process.
  • Administers prescribed medications and treatments and carries out appropriate preventive and rehabilitative nursing procedures.
  • Offers counseling to the patient and their family to address nursing and related needs.
  • Provides health care instructions to the patient as indicated by the assessment and care plan.
  • Identifies discharge planning needs during care plan development and implements them before the patient is discharged.
  • Acts as a Case Manager when assigned by the Clinical Supervisor/Nursing Supervisor, taking responsibility for coordinating patient care within their caseload.
  • Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.
  • Collaborates with the physician to address the patient's needs and report any changes in the patient's condition, obtaining and implementing physician's orders as needed.
  • Communicates with community health professionals to coordinate the care plan.

Additional Responsibilities:

  • Participates in on-call duties as defined by the on-call policy.
  • Ensures that necessary equipment and services are arranged for.
  • Provides instruction, supervision, and evaluation of home health aide care every two weeks.

Position Requirements:

  • Graduate of an accredited nursing school with one to two years of recent acute care experience in a healthcare facility.
  • Current state licensure and CPR certification.
  • A bachelor's degree is preferred, with one year of home health care experience.
  • Prior management experience is not mandatory; responsibilities include supervising home health aides.
  • Strong observational, verbal and written communication skills, problem-solving abilities, and basic math skills are required, along with nursing skills as per the competency checklist.
  • The role may involve prolonged standing or walking, patient lifting and positioning, and the ability to handle supplies and equipment. There may also be significant bending, stooping, kneeling, or crouching. Adequate visual acuity and hearing are necessary to perform the required nursing tasks.

(A valid driver's license and insured automobile are required in accordance with state or organization requirements.)

Qualifications :

  • Current and unrestricted RN license

  • 1 year: Hospice Experience

  • Current BLS/CPR certification

  • Valid Driver's License
