Job description

Make a Difference in the Lives of Our Residents

We are seeking a compassionate and knowledgeable Registered Dietitian to join our interdisciplinary care team at our skilled nursing and long-term care facility. In this role, you'll play a vital part in enhancing the quality of life for our residents through nutrition planning, assessment, and education.

What You'll Do:

  • Complete nutritional assessments and develop individualized care plans for residents

  • Monitor and document residents’ nutritional status and adjust plans as needed

  • Collaborate with nursing, dietary, and medical staff to ensure comprehensive care

  • Provide in-service education and training to dietary staff on best practices

  • Ensure compliance with all applicable federal, state, and local regulations

  • Participate in care plan meetings and quality improvement initiatives

What We’re Looking For:

  • Current Registered Dietitian (RD/RDN) license in Illinois

  • Experience in long-term care or skilled nursing settings preferred

  • Strong communication, organization, and documentation skills

  • Knowledge of MDS and regulatory requirements (CMS, DOH, etc.)

  • Passion for improving the health and well-being of elderly residents
