Company Overview
Mental Health Resource Center is a not-for-profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community. Services include 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults, as well as outpatient services such as medication management, case management, and counseling.
Job Summary
The FACT-IL (FACT Lite) Case Specialist provides treatment, rehabilitation, care coordination, and recovery support services to individuals served by the FACT-IL program under clinical supervision. As a member of the multidisciplinary treatment team, the Case Specialist promotes recovery, empowerment, self-determination, and community stability for adults with serious mental illness.
 
FACT-IL operates under DCF Guidance 36 and provides a lower level of care than traditional FACT services. The program supports individuals transitioning from higher levels of care while promoting continuity of services and successful community integration.
 
Responsibilities 
  • Provide treatment, rehabilitation, and support services in accordance with DCF Guidance 36 and FACT-IL program standards. 
  • Complete assessments, evaluations, and assigned portions of comprehensive assessments. 
  • Coordinate services and develop, implement, and monitor individualized recovery plans in collaboration with individuals served and treatment providers. 
  • Advocate for individuals served to access medical, dental, housing, legal, financial, and other community resources. 
  • Provide support with activities of daily living, transportation, communication, and interpersonal skill development. 
  • Educate individuals served regarding symptoms, medications, coping skills, and mental health recovery. 
  • Maintain accurate, timely, and confidential clinical documentation and records.

  • Reduces time spent on documentation, allowing more focus on client interaction and care 
  • Transforms brief notes or session conversations into complete, clinically appropriate documentation 
  • Supports accurate, compliant documentation while allowing staff to review and finalize notes 
  • Provides helpful insights from sessions to support care planning and continuity 
  • Accessible on desktop or mobile for flexibility across different work settings 
Qualifications
  • Bachelor’s degree in Human Services or a related field required; Certified Recovery Peer Specialist (CRPS) credential preferred.
  • Experience working with adults with severe and persistent mental illness required.
  • Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.
  • Proficiency in Microsoft Office, Outlook and the use of the Internet.
  • Must meet MHRC/RBHS driver requirements, including a valid Florida driver’s license, required insurance coverage, and acceptable driving record.
  • Ability to work in a variety of settings, including homes, offices, hospitals, nursing homes, and secure units.
  • Ability to make sound, independent decisions, including responding to emergencies and high-risk situations.
  • Strong communication and interpersonal skills; ability to work effectively with individuals, families, providers, and team members.
Position Details
This position is a Full Time position (rotating weekends, on-call, and holidays).
